We now offer the Medicare Wellness Visit.  This is a new type of appointment which focuses on preventative health and is fully covered by Medicare.   It's a chance to spend some focused time talking with your doctor and ensuring you are doing everything you can to best maintain your health.

 

In order to make the most of your visit, we ask you to prepare by reviewing the following information:

 

 
How should I prepare for my appointment?

Fast: 

Please fast for at least 4 hours prior to your visit in case you need bloodwork.

Family health history:

Try to learn as much as you can about your family's health history before your appointment. The information will help you and your doctor better understand what screenings you should get and what to watch for in the future.

List of other physicians:

Bring a list of any other health care professionals that you have seen in the past year.  This list should include their specialty and phone number.

Medications:

Bring a bag with all of your prescription drugs, over-the-counter drugs, vitamins, and supplements that you currently take.
